Why Documentation Is Everything

For Applicants:
Even minor conditions matter. A single diagnosis may not qualify on its own, but a combination of symptoms and diagnoses, when well-documented, can lead to a successful claim.

For Approved Members:
Approval is not permanent. During Continuing Disability Reviews (CDRs), a lack of current documentation can result in your benefits being stopped. Staying current with your records is the best way to protect your status.

How to Properly Document Your Conditions

1. Include All Symptoms – Even the Small Ones:
Pain, fatigue, dizziness, memory issues, mood swings – no matter how minor they seem, include them all.

2. Stick to Your Treatment Plan:
Regular appointments and following medical advice show that your condition is ongoing and being actively managed.

3. Request Specific Notes from Your Doctor:
Ask your doctor to document your functional limitations – such as how long you can sit, stand, walk, lift, or concentrate.

4. Download Records from Your Patient Portal:
Collect visit summaries, test results, and specialist notes to create a detailed paper trail.

5. Keep a Personal Symptom Journal:
Use a notebook or app to track daily symptoms and limitations. This personal record can be valuable evidence.
